What are Special Needs Plans?

Special Needs Plans (SNPs) are a type of Medicare Advantage Plan that provide additional coordinated, specialized care for the most vulnerable Medicare beneficiaries. There are three different types of SNPs:

  • C-SNPs – for those with chronic conditions such as dementia, diabetes or stroke
  • D-SNPs – for those who are eligible for both Medicare and Medicaid (dual eligible) and generally live in the community.
  • I-SNPs – for those who are in institutions such as nursing homes, or who are eligible for institutional care

 

TIHP: A Special Needs Plan

Texas Independence Health Plan (TIHP) is proud to offer an Institutional Special Needs Plan (I-SNP) and Institutional Equivalent Special Needs Plan (IE-SNP) for residents in long-term care facilities or those who live at home and/or in a contracted assisted living facility in selected counties and Texas has certified that they need the type of care that is usually provided in a nursing home.

TIHP provides services in the following types of facilities:

  • Long-Term Care Facilities
  • Skilled nursing facilities (SNFs)
  • Nursing facilities (NFs)
  • Assisted living facilities (ALFs)

Texas Independence Health Plan is a HMO-SNP with a Medicare contract. Enrollment in Texas Independence Health Plan depends on contract renewal. H5015_TIHPWEB_2025
